Dr. Jacob Boehme, PT, DPT

Clinic Director, Physical Therapist

Jacob is an Arizona native who was born and raised in Mesa, AZ. He and his wife have been married for 1 year and have 2 kids, ages 7 and 5. He decided at 15 years old to become a PT when he attended physical therapy as a patient. He witnessed the influence his PT had by assisting him to return to playing sports, and he wanted to have that same impact on others. He attended PT school at Northern Arizona University in Phoenix and graduated in March 2021.

I have worked for Moore Physical Therapy for the past 10 years as an Exercise Specialist, Scribe, PT Student, PT, and Clinic Director.

I value quality time with my family and building a genuine connection with each person I meet. I strive to help others to become the best version of themselves.

He loves educating patients about their condition, encouraging them, and guiding them back to the activities they love.

He is certified in Dry Needling and has been trained in Joint Manipulations/Adjustments.

I enjoy treating all patient conditions, however, I especially enjoy treating patients who have conditions that are underserved in our healthcare system – headaches/migraines, jaw pain, SI joint pain, and neuropathy.

I love working at Moore Physical Therapy, because I align with the purpose and values in a way that allows me to work as the most authentic version of myself. I am grateful that the company uses these principles and values to guide the company’s decisions and future.

When not working, he enjoys spending time with his family, playing sports, listening to music, and playing the drums.
