These are website terms. They are not your treatment agreement, your financial responsibility form, or your consent to care. Those are separate documents you sign at the clinic, and they govern your treatment.
This site is not medical advice
Everything on this website is general education. It is written to help you understand what physical therapy does and whether it might help you. It is not a diagnosis, not a treatment plan, and not a substitute for being examined by a clinician.
Reading a page here does not make you our patient, and submitting a form does not either. A clinical relationship starts when you are evaluated at one of our clinics.
If you think you are having a medical emergency, call 911. Do not use a form on this website.
Appointment requests
Requesting an appointment, through our patient portal or by phone, is a request, not a confirmed appointment. Your appointment exists once a member of our staff has spoken with you and scheduled it.
Content and ownership
The text, photographs, logos and design of this site belong to Moore Physical Therapy or are used with permission. Please do not republish them commercially without our written consent. You are welcome to print or share a page for your own use, or to send it to someone who needs it.
Links and embedded services
This site links to and embeds services we do not control, including Google Maps, our patient portal, our payment portal and downloadable forms. We are not responsible for those services or for what they do with your information, and their own terms apply when you use them.
Accuracy
We work to keep this site accurate, including our hours, our addresses and the insurance plans we accept. Things change, and insurance networks change often. Call the clinic to confirm anything you are relying on.
Limitation of liability
To the extent the law allows, Moore Physical Therapy is not liable for indirect or consequential loss arising from your use of this website. Nothing in these terms limits any liability that cannot lawfully be limited, and nothing here affects your rights as a patient.
Governing law
These terms are governed by the laws of the State of Arizona. Any dispute about them belongs in the courts of Maricopa County or Pinal County, Arizona.

If any part of these terms is found unenforceable, the rest still applies.
