Who this covers

This policy covers moorephysicaltherapy.com, the website of Moore Physical Therapy, which operates clinics in Mesa, Tempe and Gold Canyon, Arizona.

It is not our Notice of Privacy Practices. That is a separate document about your protected health information under HIPAA, and you receive it when you become a patient. If you want a copy before then, call any of our clinics and ask.

What this site collects

Only what you type into it. This website has one form of its own: a coverage check. Appointment requests are made through our secure patient portal at patients.moorephysicaltherapy.com, a separate service that this site links to.

  • Coverage checks collect your name, date of birth, ZIP code, insurance provider, member ID, group number if you have one, and your phone and email so we can give you the answer.

We do not sell any of it. We do not trade it. We use it to contact you about your care and nothing else.

A note about the coverage form

The coverage check asks for a member ID, which is sensitive. It reaches our staff through our website host and our practice systems, and it is used to verify your benefits with your insurer. If you would rather not send it over the internet, call us instead and we will take it over the phone. That is a completely reasonable choice.

Text messages

The appointment form has a consent checkbox for text messages. It is optional. If you tick it, we may text you about scheduling and your care. You can stop at any time by replying STOP or by telling any staff member. Message and data rates may apply.

Cookies and analytics

This site uses cookies only where a feature needs them, and it embeds two third-party services: a Google Maps frame so you can find us, and a Google reviews widget. Those services set their own cookies and are governed by their own privacy policies, not by this one.

How long we keep it

Form submissions are kept as long as we need them to respond to you and to meet our record-keeping obligations as a healthcare provider. Patient records are governed by Arizona and federal law, not by this website policy.

Your choices

You can ask us what we hold, ask us to correct it, or ask us to delete anything that is not part of a medical record we are required to keep. Call the clinic nearest you and ask for the office manager.

Children

This website is not directed at children and we do not knowingly collect information from anyone under 13 through it. We do treat minors as patients, with a parent or guardian involved.

Changes

If we change this policy we will update the date at the top of the page. This version is dated August 2026.
