Which clinic is yours?

Most people choose by drive time, and that is usually the right call. The care is the same at all three: directed by a licensed physical therapist at every visit.

If the choice is close, pick the one that is easiest to get to.

• Mesa, on South Power Road. Call (480) 218-1344. Open 7am to 6pm, Monday through Friday.
• Tempe, on East Baseline Road. Call (480) 833-7879. Open 7am to 6pm, Monday through Friday.
• Gold Canyon, on South Kings Ranch Road. Call (480) 982-7948. Open 7am to 6pm, Monday through Friday. This is the closest clinic if you are in Gold Canyon or Apache Junction.

All three clinics keep the same hours: 7am to 6pm, Monday through Friday, and closed Saturday and Sunday.

What happens when you call

Someone at the front desk picks up during clinic hours. Tell them where it hurts and which clinic is closest to you.

They will ask a few questions, then get you on the schedule and tell you what to bring. If you would rather not explain it over the phone, request an appointment online (opens in new tab) through our secure patient portal instead.

Email, fax and everything else

Each clinic has its own inbox, and the pattern is simple: the city, then our domain. Mesa is mesa@moorephysicaltherapy.com, Tempe is tempe@moorephysicaltherapy.com, and Gold Canyon is gc@moorephysicaltherapy.com.

Some questions have a faster route than the phone.

• New patient paperwork: everything is on the patient forms page.
• Coverage questions: start on the insurance page, then call your clinic with anything it does not answer.
• A balance you want to clear: the pay your bill page takes about a minute.
• Working here: careers, and applications go to hiring@moorephysicaltherapy.com.
• A doctor’s office sending a referral: fax Mesa (480) 218-1356, Tempe (480) 844-8411, or Gold Canyon (480) 982-9145.

Before you pick up the phone

Self-pay patients can come directly to the clinic without a referral, as Arizona allows direct access for self-pay patients. Patients using insurance need a referral. Not sure about your coverage or referral requirements? Call your clinic and our team will verify them for you.

Spanish-speaking help is available. Dr. Caden Alexander, PT, DPT, treats patients in Spanish at our Tempe clinic.

Not sure we are the right first stop? Say what is going on anyway. If a physical therapist is not who you need first, we will tell you.

One call is all this takes

You are the person other people call when something needs handling. Which is exactly why your own thing has been waiting this long, and it was never weakness. You were busy.

It does not have to keep waiting. The longer this runs, the more of your week quietly rearranges itself around it: the shorter walk, the chair you avoid, the thing you stopped saying yes to.
