Insurance and coverage.

The short version

We are in network with more than fifty plans, including Medicare, AHCCCS and TRICARE. We also take liens, PPOs and most workers compensation claims. Se habla español.

If your plan is on the list below, you are almost certainly covered. If it is one of the plans with limits noted below, or if you cannot find it at all, call the clinic nearest you and we will check the specific plan rather than guess. That call takes a few minutes and it is the difference between knowing your cost and finding out later.

Before your first visit

Before your initial visit, we give every patient information about both their in-network and out-of-network benefits, under the criteria of the No Surprises Act. You will know where you stand before you walk in.

A note for UnitedHealthcare members: because we are out of network with United, we are unable to see United plans that do not have out-of-network benefits, including Banner plans, Community Plans, AARP Medicare Advantage and HMO Optum plans. Call your clinic and we will check your plan with you.

Do I need a referral?

Self-pay patients can come directly to the clinic without a referral, as Arizona allows direct access for self-pay patients. Patients using insurance need a referral. Not sure about your coverage or referral requirements? Call your clinic and our team will verify them for you.

Injury claims and workers compensation

We accept liens, PPOs and most workers compensation claims, and we document care thoroughly from the first visit so your records are there when someone asks for them. We can send those records wherever you direct us.

What we will not do is tell you how a claim will end. Coverage and payment sit between you, your insurer and, if you have one, your attorney. Anyone who promises you an outcome on that is guessing.
